 A community health center can outsource its billing, watch its clean claims rate improve, and still lose revenue every month on visits that never become paid claims. FQHC billing services tend to leak at the handoffs between your staff and your vendor, where the contract assigns the work to nobody. 

### **Why Community Health Center Billing Leaks Between Teams**

Billing contracts are usually written around the claim itself. The work that decides whether a clean claim exists in the first place often sits outside that scope.

The cause is structural.
